KVCS Transcript Requests

Current Students: Use this link to fill out the google form.  You must be logged in to your KVCS google account.

Alumni and Former Students: Use this link for the PDF form.

In either case, KVCS staff must receive a signed form (that includes all the pages of the request) before we can send a transcript to a 3rd party/institution/organization.  If the current student is not 18 years old, it needs to be signed by a parent/legal guardian.  Otherwise the transcript needs to be requested directly by the student.

You may print, sign and then mail/fax/scan the form to KVCS
