Medications at School
Keeping Medications in the School Office:
With few exceptions, students should not carry medication at school. Medications can be kept in the school office and administered by school personnel. Please note the following guidelines:
- Medications should be brought to the school office by the parent/guardian.
- All over-the counter medications should be in the original containers. No baggies, please.
- Prescription medications should have the pharmacy label indicating dosage, timing, and provider. Please note the expiration, as we cannot administer expired medication.
- Helpful hint: Ask your pharmacist for an extra “school bottle” if you need to divide the medication.
- School personnel cannot cut pills or tabs. Please do this before bringing in.
- The first dose of any medication should be given at home--in the rare event of a reaction.
- Schools cannot administer medications that don't have FDA approval (homeopathic remedies, supplements, etc.) without a written order from the student's medical provider.

Self-carrying Medications:
Students who need access to emergency medications may carry and self-administer them if they are developmentally able to do so. Also, in some instances, small amounts of non-emergency medications may be self-carried and administered with the consent of the school nurse or administrator. If your student needs to carry and self-administer a medication, please complete the following form. It can be filled out and submitted via email or printed and brought into school:
Immunizations
The state of Oregon requires vaccinations for all students attending public and private schools, preschools, child care facilities, and Head Start programs, unless an exemption has been requested.
Every student entering Philomath School District must supply documentation of vaccination status. This can be done by completing the Oregon Certificate of Immunization Status 
form or by asking your child's medical provider for a copy of the immunization record.
Each year, to begin school, a student must have received at least one of each of the required vaccines, unless they have an exemption already on file. All vaccine requirements (per age/grade) must be fulfilled before Oregon's designated "Exclusion Day" each year. If a student has not received all required vaccinations by Exclusion Day, the student must be excluded from school.
Students transferring from schools outside of Oregon, but within the United States, have 30 days from the date of enrollment to submit immunization records. After 30 days, a student without immunization documentation must be excluded from school.
screenings
Health screenings are simple evaluations that help to identify difficulties that may interfere with learning. With the help of our community partners, the Philomath School District provides access to dental, vision, and hearing screenings for all of its students as they enter kindergarten. Screenings continue each year, along the following schedule:
Dental: K-5 grades
Vision: K-5 grades
Hearing: K, 1, and 3 grades
Parents can opt-out of screenings when they register their students at the beginning of each school year or at any time afterward by notifying the school office.
Crutches, Splints, Casts and Boots
If your student has suffered an injury and needs to use medical equipment at school, please provide us with a note from your child's health care provider. The note should include:
- Reason for medical equipment
- Any activity restrictions and for how long
- Will extra assistance be required?
- For crutches: Has student been educated in their use?
Because improper use of medical devices can be harmful to both the the student and to others, students without a provider's note can not be allowed to use these devices at school.
Medical Conditions
Diabetes
Your student is likely eligible for: 1) a 504 plan that will outline needed accommodations and 2) an Individual Health Plan that will specify emergency responses.
At the beginning of each school year:
- Plan to meet with the school nurse and the 504 team to discuss student medical and educational needs.
- Provide school with current diabetes treatment orders from student's medical provider. Make sure that orders are specific to the mode of insulin administration (pen or pump).
- Provide school with all necessary supplies: glucose testing meter, strips, lancet, ketone strips, and alcohol swabs.
- Provide school with carbohydrate and protein snacks.
- Provide school with emergency glucagon.

Allergies
By the beginning of each school year:
- Contact school about the student's allergies and needs
- Provide school with emergency action plan from health care provider, if one was given
- Provide school with epinephrine

Seizures
By the beginning of the year, please provide us with:
- A school emergency action plan, if your student's health care provider has given you one
- Any prescribed emergency medication (like Diastat or Midazolam nasal spray)
If your student has not been given a school emergency action plan by their health care provider, we suggest the following:
Covid-19 Guidelines
From the Oregon Health Authority:
If you are infected with COVID-19 you do not need to isolate for a set number of days. However, you should be aware that you can get others sick. If you have COVID-19:
- Stay home until you have not had a fever for 24 hrs without using fever reducing medication and other COVID-19 symptoms are improving.
- Avoid contact with high-risk individuals like people who live in congregate care facilities or people with with immunocompromising conditions for 10 days.
- Mask when you are around other people in the 10 days after you become sick or test positive.
